Download, print, and fill out the TMS New Patient Packet. Please scan and email the packet to ins_dept@thehealthcenters.com  to reduce your wait time before your appointment. 

If you are unable to do this, please complete the online questionnaire or bring the paperwork with you.  Bringing the completed paperwork at your appointment time will increase your wait in the office, so please show up 15 minutes early with your completed documents, ID, and insurance card.
